RIDOH Launches New Electronic Portal for Animal Bites/Rabies Case Reporting and Rabies Testing

The Rhode Island Department of Health (RIDOH) has developed a new electronic case report form for animal bites and potential rabies exposures and a new secure electronic submission form for rabies testing at RIDOH’s Rhode Island State Health Laboratories (RISHL).

Both forms utilize the secure REDCap platform. We encourage using these new forms, which are streamlined, more user friendly, and contain the same information required on the paper/PDF versions of the forms. 

Animal bites/rabies reports will be accepted via REDCap or through the traditional method of secure email/fax until March 2025. After that time, only REDCap submissions will be accepted.
